Patients who show up for their appointment 15 minutes or more late (at home or in clinic) are deemed a “no-show”. A $50 charge will be issued via the card on file used to book the services.
Due to the personal and tailored nature of our services, all deposits and payments are non- refundable nor transferable to other services or clients.
If for some reason your IVF/IUI cycle is canceled for any reason, with documentation from your provider documenting the cancellation due to medical reasons, we will extend a one- time courtesy and allow your deposit to be used within 6 months for the same booked package or a similar service.
To the Point cares about the wellbeing and safety of all of our clients. To ensure we are maintaining sanitary and health regulation standards, all items purchased/administered as part of the services rendered are final sale and non-refundable.
If there is a circumstance in which an IV service cannot be completed due the inability to establish an IV line; the patient will be billed a service fee charge of $90 and all other fees paid will be returned within 48hrs of the appointment cancellation.
To the Point is NOT responsible for any injection supply costs and/or prescription medication costs as prescribed to the patient by their provider. To the Point will supply basic PPE to their employees as a part of the fee paid for services rendered, but all other supplies and medications, as well as their costs, are the sole responsibility of the patient.
To ensure a safe and successful treatment, legal guardians must accompany the minor(s) for all services provided.
Sales tax is applicable to all appointments and services rendered.
All rendered services are NON-REFUNDABLE due to the nature of the services and supplies provided.
Appointment cancellations within 4 hours of the scheduled appointment time and/or appointment no shows (tele-med or in person) are subject to the full fee for the appointment service requested. Appointment cancellations 5 hours or more prior to the scheduled timeframe (tele-med or in person) are refundable up to 75% of the original fee.
Membership cancellation must be completed within 30 days prior to your next billing cycle. Cancellations received after the 30 day requirement will go in to effect for the next billing cycle.
Rowan Family Practice, LLC always operates with your best interests and safety at heart; if we feel your needs are outside of our scope of practice it may require referral to another provider and/or facility for your care. We will attempt to triage your needs at the time of scheduling to ensure prompt, appropriate, and safe care; but due to the nature of medical practice and it's inherent limitations, providers may not be able to determine if safe at-home services can be provided until an actual assessment is performed.
